Care Manager

Mental Health organization is excited about hiring an experienced Tailored Care Manager for community-based work. Must possess strong written and verbal communication skills. Have a history of working with underserved communities and be comfortable with mental health and/or physical health populations.

Duties:

• Provide case management services to individuals and families in need
• Conduct assessments to determine client needs and develop individualized service plans based on the unique needs of the consumer.
• Care Coordination: Coordinate and monitor the delivery of services to clients and collaborate with other service providers and professionals, such as social workers and healthcare providers, to provide comprehensive care
• Community Engagement: Assist clients in accessing community resources, support services, and connecting with natural supports and health promotion activities
• Advocate for consumers' rights and ensure their needs are met
• Maintain accurate and up-to-date case records
• Crisis Management: providing 24-hour crisis response and support
• Educating clients on self-managment, self-advocacy, and wellness programs

Requirements:

• Bachelor's degree in social work, psychology, or a related field
• Experience in case management or a related field

Job Type: Full-time

Pay: $60,000.00 - $62,000.00 per year
