Job Title: Clinical Social Worker (LCSW / LICSW)Location: Hyannis, MAJob Type: Contract – 26 WeeksShift: 8:00 AM – 4:00 PMSchedule: Rotating Days with Rotating WeekendsHours: 40 Hours/WeekOccasional holiday coverage requiredAbout the Role:Seeking an experienced Clinical Social Worker to provide psychosocial support, crisis intervention, and discharge planning services within an acute care hospital setting.Key Responsibilities:Conduct psychosocial assessments for patients and familiesProvide counseling, advocacy, and crisis intervention servicesSupport patients with mental health, substance abuse, and social needsDevelop individualized discharge plansCoordinate referrals for rehabilitation, long-term care, and community servicesAssist with financial assistance and community resource coordinationCollaborate with interdisciplinary healthcare teamsMaintain accurate EHR documentationParticipate in quality improvement and departmental initiativesRequirements & Preferred Skills:Master’s Degree in Social Work (MSW) requiredActive Massachusetts LCSW or LICSW license requiredCPI/MAB certification required within 1 month of hireStrong EHR and computer skills requiredAcute care hospital experience preferredExperience with crisis intervention and discharge planning preferredBehavioral health and substance abuse counseling experience preferredStrong communication, organization, and multitasking skillsAbility to work independently in fast-paced environmentsContact Information:For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.Company Description:About Navitas Healthcare, LLC certified WBENC and one of the fastest-growing healthcare staffing firms in the US providing Medical, Clinical and Non-Clinical services to numerous hospitals.
