We are seeking a motivated and organized Hybrid Care Coordinator to support both our Group Home and HCBS (Home and Community-Based Services) programs.

Position Overview
The Hybrid Care Coordinator plays an active role across both our Group Home operations and HCBS services. This position works closely with leadership to ensure services are delivered in alignment with DDD requirements and agency standards.
This is a balanced role that includes administrative coordination, compliance oversight, field visits, and team support.
Key Responsibilities
Group Home Support
- Assist with Group Home audits alongside the Residential Program Manager
- Track and follow up on required staff certifications and compliance documentation
- Support Group Home leadership when documentation, training, or operational concerns arise
- Help coordinate staff trainings and structured communication when additional support is needed
- Maintain organized and audit-ready records
- Identify compliance gaps and escalate concerns appropriately
HCBS Support
- Attend and participate in PCSP (Person-Centered Service Plan) meetings
- Conduct or attend required in-home visits
- Follow up with providers regarding documentation and compliance requirements
- Support maintenance of accurate and organized member files
Compliance & Documentation
- Ensure documentation meets DDD and agency standards
- Maintain organized member and staff files
- Track deadlines and required follow-ups
- Assist with audit preparation and documentation requests
- Promote accountability and consistency across programs
What We’re Looking For

Experience in healthcare, behavioral health, residential services, or DDD/HCBS settings is preferred.
Reliable transportation is required, as this role includes travel to Group Homes and in-home meetings.
