- Provide all aspects of care management such as planning, coordination, assessment of health, and psychosocial status of the members.
- Identify health, environmental, and psychosocial risks for assigned members and recommend appropriate interventions.
- On-going assessment of the effectiveness of the Member's plan of care.
- Assist interdisciplinary team with crisis intervention as needed.
- Assist with community resource identification, including entitlements such as food stamps, housing applications, HEAP/SCRIE etc.
- Identify issues/concerns of Social Determinates of Health.
- Connect members to community resources that can assist them.
