Are you a clinically skilled, relationship-driven skilled nurse who thrives in a dynamic, community-facing role? Agape Hospice & Palliative Care is seeking a Clinical Community Liaison to support referral partners and patients throughout Los Angeles and Long Beach area.

Based out of our Torrance office (with excellent parking), we are a clinician-owned, Joint Commission–accredited hospice known for exceptional patient care, strong teamwork, and a supportive work culture.

This role is ideal for a skilled nurse who combines clinical knowledge with strong communication skills and enjoys building professional relationships in the community. You will serve as a key bridge between referral sources, patients, families, and our interdisciplinary team—helping ensure smooth, timely, and compassionate transitions into hospice care.

About the Role

As a Clinical Community Liaison, you will travel throughout Los Angeles and Long Beach area to cultivate relationships with hospitals, skilled nursing facilities, assisted living communities, and physicians. You will provide clinical education, evaluate potential hospice referrals, and support the admissions process in collaboration with our intake and clinical teams.

Your Responsibilities Include:

  • Building and maintaining strong relationships with referral partners throughout Los Angeles and Long Beach area
  • Providing education on hospice eligibility, services, and benefits to healthcare professionals and families
  • Reviewing clinical information to help determine hospice appropriateness
  • Supporting timely and seamless patient transitions from hospital or facility to hospice services
  • Collaborating closely with intake, admissions, and clinical teams to coordinate care
  • Conducting occasional bedside or facility visits to assess and support referrals
  • Representing Agape Hospice at community events and professional meetings
  • Tracking referral activity and maintaining accurate documentation in our CRM/EMR systems
  • Communicating effectively with physicians, discharge planners, case managers, and facility staff
  • Upholding hospice regulations, compliance standards, and professional ethics at all times

This is meaningful, relationship-based work where your clinical insight and communication skills directly impact patient access to compassionate end-of-life care.

Qualifications

  • Minimum of 3 years of nursing experience (hospice, home health, or SNF experience strongly preferred)
  • Experience in hospice admissions, intake, or case management highly preferred
  • Strong clinical judgment and understanding of hospice eligibility criteria
  • Excellent communication, presentation, and relationship-building skills
  • Comfortable working independently in a travel-based role
  • Reliable transportation, valid auto insurance, and clean driving record
  • Familiarity with Los Angeles and Long Beach area healthcare networks preferred
  • Bilingual in Spanish highly preferred
