* What You’ll Do
- Conduct in-home assessments with frail and vulnerable seniors
- Develop individualized care plans tailored to client needs
- Coordinate essential services and resources
- Provide ongoing follow-up and monitoring
- Collaborate with an interdisciplinary team of professionals
* Who We’re Looking For
- A compassionate professional with experience supporting family caregivers and individuals with physical and/or cognitive limitations
- A Bachelor’s degree in Human Services is preferred, but candidates with 2+ years of case management experience are encouraged to apply
- Must have reliable transportation for home visits within Midland County
- Basic computer skills are essential
