Case Manager (Care Coordinator)

* What You’ll Do

• Conduct in-home assessments with frail and vulnerable seniors
• Develop individualized care plans tailored to client needs
• Coordinate essential services and resources
• Provide ongoing follow-up and monitoring
• Collaborate with an interdisciplinary team of professionals

* Who We’re Looking For

• A compassionate professional with experience supporting family caregivers and individuals with physical and/or cognitive limitations
• A Bachelor’s degree in Human Services is preferred, but candidates with 2+ years of case management experience are encouraged to apply
• Must have reliable transportation for home visits within Midland County
• Basic computer skills are essential
