MAT Coordinator/Counselor

Summary:
We are seeking a dedicated and compassionate MAT Coordinator/Counselor to join our dynamic behavioral health team. In this vital role, you will coordinate medication-assisted treatment (MAT) programs and provide comprehensive counseling services to individuals struggling with substance use and mental health challenges.

Responsibilities:

• Conduct screenings, assessments, and intake evaluations for individuals participating in MAT and substance use disorder treatment programs
• Provide counseling, guidance, referrals, and recovery support services to program participants
• Develop individualized treatment plans, progress notes, documentation, and discharge planning materials
• Coordinate MAT orientation and treatment services with healthcare providers, community partners, and multidisciplinary care teams
• Monitor attendance, participation, and treatment progress within recovery programs
• Review client records and treatment participation for ongoing care planning and program eligibility consideration
• Provide continuing care planning and recovery support assistance
• Intervene appropriately during crisis situations and provide client support as needed
• Maintain accurate data, reports, and documentation within electronic healthcare record systems and other required platforms
• Perform additional duties and special projects as assigned

Job Types: Contract, Full-time

Pay: Up to $37.00 per hour
