Position: Social Worker
Location: Brooklyn, NY
Schedule: Full-Time | Monday–Friday, 9:00 AM – 5:00 PM
Salary: $60,000 – $65,000 annually

We are seeking an experienced and empathetic Social Worker to join our team in Brooklyn and provide essential support, guidance, and advocacy for residents and their families.
Key Responsibilities
- Conduct resident and family interviews to develop comprehensive social histories and individualized care plans
- Partner with families to create programs that support residents’ emotional, social, and psychological well-being
- Assist with discharge planning, financial resources, and coordination of community support services
- Collaborate with an interdisciplinary team—including nursing, administration, and medical staff—to ensure holistic care
- Advocate for residents by addressing social and emotional needs and maintaining clear, effective communication
- Master’s Degree in Social Work (MSW) preferred
- Bachelor’s degree required in Social Work, Sociology, Psychology, Special Education, Rehabilitation Counseling, or a related field
- Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals
- Experience in a Skilled Nursing Facility (SNF) strongly preferred
- Strong interpersonal and communication skills with the ability to build trust and rapport
