We are seeking a dedicated and experienced Home Care Coordinator/Case Manager to join our team. The Home Care Coordinator/Case Manager will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary services and resources to improve their quality of life. As well as scheduling caregivers, EVV managing via HHA Exchange, staffing, visiting clients when needed, completing intake and on call on rotation bases. The ideal candidate possesses previous Coordinator experience.

Duties

Assessing Patient Needs: Conducting through assessments of patients’ health conditions and needs within their homes to develop personalized care plans.

Developing Care Plans: Creating individualized care plans based on the assessment data and assign

Coordinating Care: Facilitating communication and coordination between patients, families, and healthcare providers to ensure continuity of care.

Monitoring Progress: Regularly monitoring patients’ progress and adjusting care plans as necessary to meet their changing health needs.

Documenting Care: Ensuring accurate documentation of all patient care activities, including assessments, care plans, and progress notes.

Compliance with Regulations: Ensuring compliance with federal, state, and local regulations governing home health care services.

EVV: managing call dashboard, reminding caregivers to clock in and out on timely manner, obtaining timesheets as needed.

Scheduling: adequately staffing case load and call outs.

Visiting clients: to complete intake assessment and paperwork, as well as following up on care delivery and services obtained.

Skills:

Must have previous Home Care Coordinator experience.

Must have HHAexchange experience.

Assessment Skills: The ability to conduct through assessments of patients’ health conditions and needs .

Communication: Strong verbal and written communication skills to effectively interact with patients, families, and healthcare providers

Coordination: Coordinating various aspects of patient care.

Time Management: Managing time effectively to handle multiple patients and tasks efficiently.

Flexibility: Being adaptable to changing patient needs and unexpected situations
