Overview
Join our compassionate healthcare team as a Hospice Social Worker, where you will play a vital role in providing emotional support, counseling, and case management to patients and their families during one of life's most sensitive times. You will collaborate with interdisciplinary teams to ensure comprehensive, patient-centered hospice care that promotes dignity, comfort, and quality of life.

Duties

  • Conduct thorough patient assessments to evaluate emotional, social, and behavioral health needs within hospice settings.
  • Develop personalized care plans that incorporate social work best practices, including crisis intervention and case management strategies.
  • Facilitate communication between patients, families, medical teams, and community resources to coordinate comprehensive hospice care.
  • Document all clinical encounters accurately in electronic medical records (EMR) systems while adhering to HIPAA regulations for medical documentation and records management.
  • Assist with intake procedures for new patients, ensuring seamless integration into hospice services.
  • Support families through grief counseling and bereavement services while maintaining sensitivity to cultural and individual differences.

Experience

  • Proven experience in hospice care or palliative settings with a strong understanding of end-of-life issues.
  • Background in social work or mental health counseling with expertise in child & family counseling, behavioral health, or clinical research.
  • Experience working with diverse populations including seniors, individuals with disabilities, dementia or Alzheimer’s disease, PTSD care, autism spectrum disorders, developmental disabilities, or post-acute care environments such as nursing homes or assisted living facilities.
  • Knowledge of crisis management techniques and familiarity with EMR systems used in inpatient or outpatient settings.
  • Ability to perform diagnostic evaluations and conduct intake assessments effectively.
  • Strong communication skills for collaborating within multidisciplinary teams and engaging with patients’ families during emotionally charged situations.
