Our residential programs break the cycle of abuse using treatments that restore hope and successfully transition children to more permanent, community or family-based settings. Maryhurst's community and transitional living programs offer tools that prepare survivors of abuse and neglect for better, more hope-filled futures. And lastly, the team at Maryhurst Renewal partners with children, adults and families who are hurting to find the right combination of support, ultimately clearing barriers to success at home, at school, and in the community. Renewal provides one-on-one, group and family counseling in office, home or school-based settings.


The Care Coordinator facilitates and supervises the treatment planning process for their clients and serves as a liaison and representative for child, family, and outside agencies.

  • The Care Coordinator monitors effectiveness and efficiency of services for Quality Assurance.
  • The Care Coordinator interfaces with other community resources such as legal, education and medical systems, as well as connects all services within the Maryhurst system for individual clients (therapy, medical, psychiatric, education, spiritual, etc). The Care Coordinator also assesses client/family's needs and makes referrals to necessary resources.
  • The Care Coordinator ensure that the client is in most appropriate, least restrictive setting and has clear goals toward permanency.

Requirements

Bachelor's degree in Human Services or related field. Minimum one year experience working with family and children's services or in a case management position. Ability to facilitate the treatment process and collaborate with the treatment team effectively. Must have strong organizational, assessment, assertiveness, and documentation skills.

Salary Starting: $44,900
