Job Overview
We are seeking a dedicated and compassionate Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or equivalent mental health professional to join our dynamic team. In this role, you will provide comprehensive behavioral health services, including clinical counseling, patient assessment, and case management, to individuals across diverse age groups and backgrounds. Your expertise will help clients navigate mental health challenges, behavioral issues, and life transitions while ensuring adherence to HIPAA regulations and maintaining accurate medical documentation. Responsibilities
- Conduct thorough patient assessments to determine mental health needs and develop tailored treatment plans.
- Provide individual and group therapy sessions utilizing evidence-based approaches such as behavioral therapy, applied behavior analysis, psychodynamic therapy, and Christian counseling.
- Implement behavioral health interventions focused on improving patient outcomes and promoting emotional well-being.
- Manage intake processes, including initial evaluations, documentation, and coordination of care with other healthcare providers.
- Maintain detailed medical records in compliance with HIPAA regulations, ensuring accuracy and confidentiality of all patient information.
- Engage in case management activities to support patients’ ongoing needs within home and community settings, especially for aging populations or those with geriatrics concerns.
- Collaborate with multidisciplinary teams to facilitate diagnostic evaluations, treatment adjustments, and discharge planning.
Requirements
- Valid licensure as an LSW (Licensed Social Worker), LPC (Licensed Professional Counselor), or equivalent credential in the relevant state or region.
- Proven experience in behavioral health settings, including patient care, mental health counseling, and social work.
- Strong knowledge of medical terminology, medical records management, and HIPAA compliance standards.
- Familiarity with clinical counseling modalities such as behavioral therapy, applied behavior analysis, psychodynamic therapy, and Christian counseling.
- Experience working with diverse populations addressing aging issues or geriatrics is highly desirable.
- Excellent communication skills for conducting diagnostic evaluations, patient assessments, and delivering effective individual or group counseling sessions.
