Job Title: Part-Time Licensed Master Social Worker (LMSW)Location: New York, NY 10065Duration: 27 WeeksScheduleVariable shifts: Monday: 9:30 AM – 4:00 PMTuesday: 9:00 AM – 5:00 PMWednesday: 10:00 AM – 3:00 PM24 hours per week (3x8 schedule across Monday–Wednesday)Position SummaryWe are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to provide patient-centered social work services in a healthcare setting. The ideal candidate will assess patient needs, support care planning, and help connect patients and families with appropriate resources to enhance overall care outcomes.Key ResponsibilitiesConduct interviews with patients and families to assess social, psychological, and environmental needsReview medical records and collaborate with care teams to determine appropriate interventionsProvide individual and/or group counseling to patients and families facing personal or environmental challengesConnect patients and families with community resources, including financial assistance, counseling, rehabilitation, and medical support servicesPerform discharge planning, including coordinating placement in rehabilitation, extended care, or specialized facilities when neededParticipate in interdisciplinary team meetings and communicate social and emotional factors affecting patient careSupport care teams in addressing barriers to treatment and recoveryContribute to quality improvement initiatives and program developmentRequired QualificationsMinimum 1 year of experience as a Licensed Master Social Worker (LMSW)Strong knowledge of psychosocial assessmentsExperience in medical social work within a hospital or outpatient settingValid Licensed Master Social Worker (LMSW) license issued by New York State Education Department (NYSED)EducationMaster’s Degree in Social Work (MSW)For more details reach at jknox@navitashealth.com or Call / Text at 732-791-4827.About Navitas Healthcare, LLC: It is a certified WBENC and one of the fastest-growing healthcare staffing firms in the US providing Medical, Clinical and Non-Clinical services to numerous hospitals.
