House Calls Clinical Care Coordinator (Medical Doctor)

Qualifications:

• Medical Doctor (MD) with a valid and active medical license.
• Previous clinical or patient care experience preferred.
• Familiarity with Electronic Health Record (EHR) systems and medical documentation.
• Knowledge of care coordination, patient services, and healthcare operations.
• Strong communication and interpersonal skills when interacting with patients and healthcare teams.
• Ability to manage multiple tasks, coordinate schedules, and support clinical workflows.
• Strong attention to detail and ability to maintain accurate patient records.
• Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.
• Ability to work in a fast-paced healthcare environment and collaborate effectively with multidisciplinary teams.

Key Responsibilities:

• Oversee and review patient care for house call patients.
• Assess medical records and develop individualized care plans.
• Coordinate patient visits with House Call Team.
• Reconcile medications and coordinate prescriptions with pharmacies.
• Review labs, diagnostics, and order tests or referrals as needed.
• Communicate with patients, caregivers, and healthcare teams.
• Ensure compliance with clinical standards and organizational policies.
• Monitor and address care gaps for non-compliant or high-risk patients.
