Qualifications:
- Medical Doctor (MD) with a valid and active medical license.
- Previous clinical or patient care experience preferred.
- Familiarity with Electronic Health Record (EHR) systems and medical documentation.
- Knowledge of care coordination, patient services, and healthcare operations.
- Strong communication and interpersonal skills when interacting with patients and healthcare teams.
- Ability to manage multiple tasks, coordinate schedules, and support clinical workflows.
- Strong attention to detail and ability to maintain accurate patient records.
- Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.
- Ability to work in a fast-paced healthcare environment and collaborate effectively with multidisciplinary teams.
Key Responsibilities:
- Oversee and review patient care for house call patients.
- Assess medical records and develop individualized care plans.
- Coordinate patient visits with House Call Team.
- Reconcile medications and coordinate prescriptions with pharmacies.
- Review labs, diagnostics, and order tests or referrals as needed.
- Communicate with patients, caregivers, and healthcare teams.
- Ensure compliance with clinical standards and organizational policies.
- Monitor and address care gaps for non-compliant or high-risk patients.
