Job Summary
We are seeking a dedicated and compassionate PRN Licensed Master Social Worker (LMSW) to join our dynamic Hospice team. Your expertise will help support patients and their families through comprehensive assessments, counseling, case management, and crisis intervention .Our interdisciplinary team supports patients and families with dignity, respect, and empathy during life’s most important moments.
This position offers flexible scheduling to meet the needs of our diverse patient population while ensuring high-quality, person-centered care.The LMSW provides psychosocial support to patients and families, assists with care planning, and ensures emotional, social, and community needs are addressed throughout the hospice journey.

Key Responsibilities
- Conduct psychosocial assessments for hospice patients and families
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Provide counseling and emotional support related to end-of-life issues, grief, and bereavement
- Assist with advance care planning and resource coordination
- Connect patients and families with community resources and support services
- Participate in IDT (Interdisciplinary Team) meetings and documentation in compliance with regulatory standards
- Provide crisis intervention as needed
Qualifications
- Current Licensed Master Social Worker (LMSW) license in the state of Texas
- Minimum of 1 year of healthcare experience (hospice or home health preferred)
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Reliable transportation and valid driver’s license
Pay: $55.00 - $75.00 per hour
