GEORGETOWN NURSING AND TRANSITIONAL CARE - GEORGETOWN, TX

At Wellsential Health, we’re looking for a compassionate, detail-oriented Licensed Social Worker (LSW) to join our skilled nursing team. As an integral part of the interdisciplinary care team, you'll help ensure our residents’ emotional, social, and psychological needs are supported from admission to discharge.

Key Responsibilities:

• Build relationships with residents and their families to assist with admissions, care planning, and psychosocial support
• Complete initial, routine, and as-needed psychosocial assessments and participate in care plan meetings
• Assist residents and responsible parties with advance care planning and end-of-life discussions
• Facilitate referrals for vision, hearing, dental, or behavioral health services when needed
• Provide guidance on financial or social challenges, working with the Business Office Manager as necessary
• Address resident or family concerns and collaborate with the Administrator on conflict resolution
• Conduct suicidal/homicidal ideation assessments and perform appropriate follow-up
• Help complete the MDS and QA documentation as needed
• Round each morning prior to department meetings to check in with residents
• Serve as Manager on Duty on a rotating weekend schedule

Qualifications:

• Bachelor’s or Master’s degree in Social Work (BSW or MSW)
• Licensed Social Worker (LSW) in the state of Texas (required)
• Experience in skilled nursing facilities (SNF) or long-term care preferred
• Strong communication, documentation, and problem-solving skills
• Compassionate demeanor with the ability to support residents through complex life transitions
