- Shifts: Monday - Friday (Day Shift)
- Hybrid model: 2-3 days/week in office, remainder remote
- Occasional off-hours responsiveness required for client needs
- No fixed shift or on-call differential (salary-based role)
We are seeking a compassionate and experienced Registered Nurse (RN) to join our team as a Care Manager. In this role, you will lead client care coordination efforts, conduct in-home assessments, and develop personalized care plans to ensure exceptional service delivery. Acting as a trusted advisor to families and a leader to caregivers, you will play a critical role in enhancing client outcomes and maintaining the highest standards of care. This position blends strategic oversight with hands-on involvement in a dynamic, client-focused environment.
Key Responsibilities
- Conduct in-home assessments, develop care plans, and oversee client onboarding
- Monitor ongoing care and adjust plans to meet evolving client needs
- Act as the primary point of contact for clients and families, providing guidance and support
- Maintain clear, empathetic, and professional communication at all times
- Lead and support caregivers and staff, fostering a culture of accountability and excellence
- Build relationships with referral partners and represent the organization in the community
- Ensure compliance with standards while contributing to growth and client satisfaction
Qualifications & Experience:
- Active Registered Nurse (RN) license (preferred)
- Minimum 3-5 years of clinical nursing experience
- Experience in neurology, geriatrics, dementia care, or complex medical cases
- Prior home care or care management experience is required
- Bonus: Up to $12,000 annually (performance-based)
- Mileage & Gas Allowance: $200/month
