Overview
We are seeking a dedicated Part-Time Social Worker to join our team and make a meaningful difference in the lives of our residents. This role offers an opportunity to provide compassionate social services, support mental health and behavioral needs, and collaborate with interdisciplinary teams to enhance quality of life for seniors and individuals with disabilities.
PRINCIPLE DUTIES:
- Assist with completion of Social Service assessments.
- Coordinate care with social work job-share partner to provide continuity of care, consistent communication, and smooth transitions for residents and families
- Coordinate discharge planning, including referrals to appropriate resources with social work team, family and interdisciplinary team.
- Communicate proactively with residents, families, and external providers to ensure smooth, safe transitions.
- Attend care conferences as scheduled.
- Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse and plans intervention based on evaluation findings; counsel patients and family caregivers as needed in relationship to stress and other identified coping difficulties. Provides crisis intervention when necessary.
- Assess for and educates interdisciplinary group on any special needs related to the culture of the resident and family.
- Coordinates insurance reviews and updates with interdisciplinary team members as well as family members including delivery on Medicare documentation for end of Medicare benefit.
- Assists with Medicare/Insurance appeals.
- Educates residents and families on, and assists in, preparation of advanced directives.
- Provides information and referral services for organization residents and families/caregivers regarding practical and environmental needs.
- Provides information to residents and families/caregiver and community agencies.
- Serves as liaison between residents and families /caregivers and community agencies.
- Maintains collaborative relationships with organization personnel to support resident care.

QUALIFICATIONS:
- A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education. OR
- A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education.
- Experience in long-term, care coordination or discharge planning preferred.
- Demonstrates good verbal and written communication and organization skills.
- Possesses and maintains current state licensure.
