Title- Social Worker

Location-Greeneville, Tennessee 37311 (Hybrid)

Duration-till 06/30/2027 with Possibility of Extension

Shift hrs-8:00 AM-4.30 PM

Role Summary:
Supports primary care teams by addressing patients’ social needs, coordinating care, and connecting individuals to community and healthcare resources.

Key Responsibilities:

• Conduct social needs assessments (SDOH: food, housing, etc.) and connect patients to appropriate resources
• Maintain and update resource directories (eligibility, referrals, contacts)
• Collaborate with providers, nurses, and social workers for coordinated patient care
• Perform case management: discharge planning support, follow-ups, care plan tracking, and re-engaging patients lost to care
• Assist with specialty referrals, scheduling, and medical record coordination
• Help patients access low-cost medications and patient assistance programs
• Provide patient education and support navigation of healthcare and social services
• Document all activities in patient records and communicate with care teams

Additional Expectations:

• Participate in onboarding, team meetings, and community outreach initiatives
• Build partnerships with local agencies and identify barriers to care
