Summary

Adult Service Coordinators (ASC)/Case Managers are advocates serving people with developmental disabilities and traumatic brain injuries who are out of school. ASC's work in partnership with individuals, their families, public or family guardians, staff, provider agencies and the community to plan, coordinate, facilitate, and monitor the implementation and ongoing needs of services and supports for individuals who are eligible for services from area agencies.

Essential Functions

  • Advocates for and coordinates services on behalf of the individual and their family and guardian (if applicable) to ensure that all services are designed to promote the individual's personal development, independence and quality of life in a manner that is determined by the individual and his/her guardian/family.
  • Provides or assists individuals and families in accessing family supports, community resources and services, including providing information to families about the regional Family Council.
  • Ensures that the environment or setting in which an individual receives services promotes the person's freedom of movement, ability to make informed decisions, self-determination, safety, participation in the community, employment and connection with family and friends.
  • Provides support in helping individuals and their teams make informed decisions, managing crisis, problem solving and provides education regarding what resources are available within the state, region and community.
