Job Title: Case Manager (No direct-care)
Location: Woburn, MA
Duration: 3-months contract​ (3-months extension)

Job Description: We are seeking a compassionate and organized Case Manager to support our clients in Woburn, Massachusetts. In this role, you will conduct scheduled home visits, assess client needs, monitor progress, and ensure quality care services are being provided. This is a non-direct care position with no hands-on patient care required.
At the beginning of each month, you will receive a list of approximately 60 clients to visit. All visits must be completed by the 20th of each month, with the same schedule rotating monthly.
- Conduct comprehensive client evaluations to determine needs
- Perform follow-up home visits in assigned territories
- Complete reassessments per company standards
- Monitor client condition and report changes to management
- Evaluate progress toward care goals
- Communicate concerns with supervisors promptly
- Collaborate with internal teams to ensure client needs are met
- Maintain accurate and timely documentation
Qualifications:
- Minimum 1 year of experience visiting clients at thier home.
- Reliable transportation for local travel.
