As a Resident Care Coordinator, you will oversee daily care operations and ensure residents receive the highest standard of assisted living and long-term care. Summary of Position
· Responsible for the comfort and well-being of residents, meeting residents’ needs and respecting Resident Rights.
· Be on-call for staffing and resident emergencies. Coordinate with Administrator to share an on-call schedule and cover any call-offs or answer emergency calls from staff during your on-call nights and days.
· Coordinate all care provided by outside care partners including, but not limited to medications, physician visits, transportation and therapies. Act as a liaison with the community care partners and the community.
· Communicate with families about a resident’s care when directed by the Administrator to do so.
· Organizing and maintaining resident charts and paperwork.
· Serve as a Medication Technician and Caregiver as needed.
· Coordinate enrichment program.
· Provide care to residents as needed, as outlined in the Caregiver duties and responsibilities.
· Is friendly, cooperative, courteous, and concerned about residents' physical and mental condition and is constantly alert for their safety.
· Must be able to act responsibly and to make quick, sound decisions in emergency situations.
· In conjunction with the Administrator, assist with on-boarding and training of all new team members.
Job Knowledge:
· Must be aware of and understand the needs of the elderly.
· Must be able to recognize unusual behavior or physical condition of residents.
· Must become thoroughly familiar with all policies and procedures of the facility.

Pay: $17.00 - $20.00 per hour
