What We’re Looking For

  • MSW from an accredited program (LMSW or LCSW preferred)
  • Hospice or home health experience strongly preferred
  • Strong communication and empathy (you know how to connect with families)
  • Organized, reliable, and comfortable working independently
  • Arizona driver’s license + reliable transportation

Your Role

  • Provide emotional and psychosocial support to patients and families
  • Conduct assessments and develop care plans
  • Participate in IDG meetings and collaborate with nurses, chaplains, and physicians
  • Assist with community resources, advance directives, and end-of-life planning
  • Support families through grief and bereavement

Compensation

  • $32–$35/hour (based on experience)
  • PRN/per-visit options available ($80–$85 per visit)
  • Mileage reimbursement
  • Performance incentives available
