Position Summary

As a Care Manager at AOASCC, you’ll play a vital role in supporting older adults as they navigate life’s transitions. You’ll help clients live safely and independently by developing individualized care plans, coordinating key services, and working hand-in-hand with families, healthcare professionals, and community partners. This is your opportunity to deliver high-quality, person-centered support that truly makes a difference.

Key Responsibilities

  • Develop and manage personalized care plans based on client assessments and needs

  • Coordinate services including hospital discharge planning, mental health support, and long-term care options

  • Collaborate with family members, healthcare providers, and interdisciplinary teams to ensure comprehensive, seamless care

  • Advocate for clients and connect them to appropriate community-based resources and supports

  • Maintain thorough, timely documentation in compliance with agency and regulatory standards

Minimum Qualifications

  • Bachelor’s degree in Human Services, Social Work, or a related field

  • At least 2 years of experience in case management or a similar social services role

  • Strong understanding of Medicaid regulations and care coordination practices

  • Excellent communication, organizational, and interpersonal skills

  • Compassionate, patient, and committed to a client-centered approach

Preferred Qualifications

  • Master’s degree in Social Work or a related discipline

  • Experience working with older adults in home care or community-based settings

  • Certification in Case Management (CCM, ACM, or equivalent)

Key Skills

  • Empathy and high emotional intelligence

  • Strong relationship-building and active listening abilities

  • Solid knowledge of elder care systems and Medicaid

  • Detail-oriented with excellent time management

  • Proven success in coordinating long-term care services
