QualificationsJob Requirements
- MSW, CSW, and/or Master’s Degree in a clinical field preferred. An equivalent combination of education and experience may substitute. Supervision of a registered nurse, social worker or other qualified person is required.
- Responsible for coordination of discharge planning services within the Utilization Review Department
- Daily audit to ensure effectiveness of clinical plans
- Verification of communication with appropriate supports and partners
- Direct discharge planning and case coordination assignments
- Provides reports and develops discharge process improvement as directed by the Director of Utilization Review
- Ability to define problems, collect data, establish facts and draw valid conclusions.
- Must be able to express or exchange ideas by means of spoken and written word.
