Home Health and Hospice Social Worker

Superior Home Health and Hospice is looking for a dedicated and compassionate Social Worker to join our team. At Superior we believe in providing our patients and families with the highest level of care and we are seeking someone who embraces excellence. Our service area includes Santa Maria, Lompoc, Santa Ynez Valley, and the 5-Cities. In order to qualify, you MUST have a Bachelor's or Master's degree in Social Work or related field (Sociology, Psychology, etc...). Home health and/or hospice experience is preferred, but if you are enthusiastic, motivated, a good team player and willing to learn, we will provide training!

Job Duties include:

• Conducting psychosocial assessments with patients and families
• Developing interventions and goals for the patient’s plan of care
• Educating and referring to community resources
• Grief support and counseling
• Clinical chart documentation in EMR
• Meeting with interdisciplinary team twice a month
• Maintaining bereavement program
