Job Category: Hospital NursingJob Description:Regions Hospital is seeking an experienced RN to join our Care Management department in an RN Care Manager position working day shifts and every 4th weekend.This position provides inpatient vs Hospital care management services for medical/surgical patients to facilitate optimal patient outcomes, supports the delivery of effective and efficient care, improve continuity of care and efficient use of resources to progress the plan of care. Focuses on developing a discharge plan to prevent readmissions and patient focused. The RN Case Manager will collaborate with the care team including, but not limited to: attending/specialty physician(s), Social Work, Utilization Review, therapies, and Nursing to ensure that an effective and timely treatment plan is acted upon, and appropriate discharge plan is implemented with a smooth transition in levels of care. Provides Care Management services on weekends and/or holiday(s) rotations as determined by leadership.Work Schedule:Day shift
Every 4th weekend
One holiday per year1.0 FTERequired Qualifications:Associate's degree of nursing with RN licensure from an accredited collegeOne (1) year working as a registered nurse.Current State of Minnesota Nursing Board RN licensePreferred Qualifications:Bachelors or Master's degree of NursingThree years' experience. Experience performing case management in Home Care, hospital, Post-Acute Care, discharge planning, care and/or quality improvement.Certified Case Manager through Commission for Case Manager Certification (CCMC) or eligibility to sit for CCM exam within 2 years of hire.About Us:At HealthPartners we believe in the power of good - good deeds and good people working together.
