Case Manager - Berkemeier Living Center
Our Case Manager is a key member of our interdisciplinary care team, which is focused on providing person-centered care that is highly individualized and attuned to the specific needs and preferences of those we serve. Day-to-day responsibilities involve resident assessments and interventions, interface with other interdisciplinary team members, supervision of the Residency Coordinator and education of residents, family members, and staff. A love for working with Seniors is a must, as is having a passion for bringing healing and hope to those we serve. Successful incumbents will possess a nurturing disposition, strong interpersonal skills, and the compassion it takes to get the job done while building strong bonds and relationships.
Responsibilities
- Assist with pre-admission activities.
- Assists with new resident adjustment to the facility.
- Provides ongoing professional social work services to each resident for the duration of their residency.
- Provides oversight to the Residency Coordinator:
- Documents appropriately and in a timely manner in resident charts (which may include specific 1:1 visits, group work, mental health updates and psychosocial interactions), including a resident summary/update at least once every six months.
- Assists in identifying and carrying out discharge plans
- Participates in relevant meetings.
- Provides education to registrant/family/staff.
- Works with the psychiatrist/psychologist to insure the mental health and well-being of the residents.
- Maintains and develops work-related knowledge and skills through continuous learning activities, attendance at in-services and workshops, etc.
- Completes special projects/tasks in support of effective department functioning as assigned by supervisor.
Education
- Bachelor's degree in social work required.
- Master's degree in social work is preferred.
Experience
- 3-5 years of professional experience in a long-term care/assisted living setting preferred. Will consider exceptionally strong candidates who have a LMSW or LCSW without 3-5 years of experience.
Compensation: $56,000-$64,000 per year
