Pay Package

Role: Hospital-Based Social Worker

Requirements

• 2+ years recent specialty experience
• Georgia Licensed Clinical Social Worker (LCSW) license required
• Acute hospital experience required
• No local candidates within 50 miles

Job Description

• Provide psychosocial assessments and crisis intervention for hospitalized patients
• Develop discharge plans and coordinate post-hospital care/services
• Collaborate with physicians, nurses, and care teams for safe transitions of care
• Educate patients and families on community resources and support systems
• Assist with insurance, placement, and referral processes (SNF, rehab, home health)
• Maintain accurate documentation in EMR and ensure compliance with hospital policies

Job Types: Contract, Travel healthcare

Pay: $48.00 - $52.00 per hour

Experience:

• Social worker: 2 years (Required)
