Position Overview:
We are seeking a dedicated and compassionate Care Coordinator to support individuals receiving services at our Rural Crisis Center. This position operates within a Certified Community Behavioral Health Clinic (CCBHC) model, focusing on timely access to care, person-centered treatment planning, service coordination, and continuity of care for individuals experiencing behavioral health crises. The Care Coordinator plays a vital role in connecting consumers to needed services, promoting recovery, and supporting successful transitions across levels of care.
Qualifications:
- Bachelor’s degree required
- Current valid driver’s license for the past three years with an excellent driving record
- Eligibility for coverage under center liability insurance and individually held vehicle liability insurance
- Strong communication, organization, and documentation skills
- Ability to work effectively with individuals experiencing mental health and substance use crises
- Ability to coordinate care across multiple providers and community resources
- Must successfully pass a background check
Responsibilities:
- Gather relevant clinical and social information from adults receiving crisis services
- Develop and maintain person-centered plans of care that support stabilization, recovery, and long-term wellness
- Coordinate services and establish partnerships with community providers, hospitals, housing resources, and other support agencies
- Assist consumers in accessing benefits, treatment services, and community resources they are entitled to receive
- Support discharge planning and continuity of care following crisis stabilization
- Advocate for consumers and help reduce barriers to treatment and recovery services
- Maintain accurate, timely, and confidential documentation in compliance with agency standards and CCBHC requirements
- Collaborate with multidisciplinary team members to ensure high-quality, coordinated care delivery
ALL candidates must be able to type and have basic computer skills, possess a valid AL driver’s license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance with a clear background check.
