Position Overview:

We are seeking a dedicated and compassionate Care Coordinator to support individuals receiving services at our Rural Crisis Center. This position operates within a Certified Community Behavioral Health Clinic (CCBHC) model, focusing on timely access to care, person-centered treatment planning, service coordination, and continuity of care for individuals experiencing behavioral health crises. The Care Coordinator plays a vital role in connecting consumers to needed services, promoting recovery, and supporting successful transitions across levels of care.

Qualifications:

Bachelor’s degree required
Current valid driver’s license for the past three years with an excellent driving record
Eligibility for coverage under center liability insurance and individually held vehicle liability insurance
Strong communication, organization, and documentation skills
Ability to work effectively with individuals experiencing mental health and substance use crises
Ability to coordinate care across multiple providers and community resources
Must successfully pass a background check

Responsibilities:

Gather relevant clinical and social information from adults receiving crisis services
Develop and maintain person-centered plans of care that support stabilization, recovery, and long-term wellness
Coordinate services and establish partnerships with community providers, hospitals, housing resources, and other support agencies
Assist consumers in accessing benefits, treatment services, and community resources they are entitled to receive
Support discharge planning and continuity of care following crisis stabilization
Advocate for consumers and help reduce barriers to treatment and recovery services
Maintain accurate, timely, and confidential documentation in compliance with agency standards and CCBHC requirements
Collaborate with multidisciplinary team members to ensure high-quality, coordinated care delivery

ALL candidates must be able to type and have basic computer skills, possess a valid AL driver’s license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance with a clear background check.
