Job Title: Licensed Clinical Social Worker Profession: Social Services Specialty: LCSW (Licensed Clinical Social Worker) Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (8:00 AM - 4:30 PM) Experience: Minimum 1 year required License: NC license required Certifications: BLS required, LCSW required Must-Have: EPIC experience required COVID-19 vaccine required (medical/religious exemptions only) Flu vaccine required (medical/religious exemptions only) Description: This position will specialize in SNF placement and home health/DME arrangement for adult neurology patients, primarily those who have experienced stroke and seizures. Responsibilities include disposition planning, addressing social determinants of health, and assisting with issues such as domestic violence, substance abuse, and homelessness. The role may involve opportunities to assist with other patient populations as needed. Clinical social workers handle crisis intervention, case management, and care coordination to ensure continuity of care. Experience with patient assessments and the ability to develop tailored care plans is essential. Familiarity with discharge planning and community service resources is necessary.
