Distinguishing Characteristics:
The Housing Case Manager provides intensive community-based case management services to individuals residing in the transitional housing program. The Housing Case Manager works directly with participants to support housing stability, recovery, employment readiness, life skills development, and community reintegration. This role includes frequent client contact, transportation assistance, advocacy, and coordination of services to ensure participants successfully transition to independent living. The Housing Case Manager works closely with program leadership, clinical staff, community partners, and external service providers to remove barriers and promote long-term stability.

Supervision/ Competency Evaluations:
Supervision and competency evaluations are provided through facility monitoring activities, direct observation, staff meetings, in-services, management meetings, individual meetings, Employee Improvement Process, reporting, interactions, strategic planning, outcomes, and annual competency
review.
Work Environment:
Work is sedentary and ambulant occasional physical exertion (lifting 30 or more pounds, walking, standing, etc.) ability to support patient weight in case of emergency or disability requiring assistance. Must be able to see, stoop, sit, stand, bend, reach, and be mobile (whether natural or with accommodation). Quality of hearing (whether natural or with accommodation) must be acceptable. Must be able to communicate both verbally and in writing. Must be able to relate to, work with mentally and physically ill, disabled, emotionally upset, hostile patients as needed. Must exhibit ability to display coping skills to deal with multiple situations. Risk of exposure to infections, blood borne pathogens and other potentially infectious materials or contagious diseases. For this reason, "Universal Precautions" must always be followed. Understand, support, and comply with the established workplace violence, ADA, EEOC, and show a commitment to worker safety and health and client safety. Subject to work schedule and shift changes.
Education/Qualifications: Bachelor’s degree preferred, High School Diploma or GED at minimum

License/Certification: Valid State driver’s license in good standing; LADAC/LADAC eligible
Special Requirements: Thorough knowledge and application of all patient privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule, 42 Code of Federal Regulations Part 2, and Confidentiality of Alcohol and Drug Abuse Patient Records Regulation.
Essential Duties and Responsibilities:
Client Case Management
- Provide intensive community-based case management services to program participants.
- Conduct initial assessments and ongoing service planning addressing housing, employment, healthcare, recovery support, and life skills.
- Develop individualized case plans with measurable goals and timelines.
- Meet regularly with participants to monitor progress and address barriers to stability.
- Assist participants in developing independent living skills, including budgeting, time management, and household management.
Transportation Support
- Provide transportation assistance for participants to essential appointments including:
- Employment interviews and job sites
- Medical and behavioral health appointments
- Recovery support meetings
- Court or probation appointments
- Social service agencies
- Ensure transportation is provided safely according to federal and state laws and in compliance with program policies.
Housing Stability Support
- Assist participants with securing permanent housing opportunities.
- Support applications for housing programs, rental assistance, and community resources.
- Help participants maintain program compliance and housing expectations.
Resource Coordination
- Connect participants to community resources such as:
- Behavioral health services
- Substance use treatment
- Employment and vocational training
- Educational opportunities
- Benefits and entitlement programs
- Serve as an advocate for participants when interacting with external service providers.
Documentation & Compliance
- Maintain accurate, timely, and confidential case records.
- Document case management services, transportation activities, and client progress.
- Complete required program reports, progress notes, and service plans in accordance with agency and funding requirements.
Continuing Education & Professional Licensing/Certification Requirements:
Employees are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, employee is expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. Employee must attend mandatory education programs and maintain current professional license, if applicable, in good standing.
Pay: $17.00 - $22.00 per hour
