Summary
Reporting to the Palliative Care Clinical Manager, Pediatric Palliative Care Social Worker is responsible for providing ongoing psychosocial assessments and advocate for interventions that alleviate burden and facilitate coping for assigned pediatric palliative care patients and their families. The Pediatric Palliative Social worker participates in family support, teaching, counseling, education, and advanced care planning in collaboration with the pediatric palliative care interdisciplinary team.
What You'll Bring
- Master's Degree in Social Work, LICSW or LCSW required.
- Demonstrate experience working with children and families in clinical capacity.
- Demonstrate experience working with life-threatening conditions and chronic disease.
- One year work experience in hospice and/or palliative care preferred.
- Experience in the use of electronic medical record preferred.
- Strong verbal and written, communication skills with patients, caretakers and providers.
- Demonstrated ability to work independently with minimal supervision.
- Demonstrated Ability to work as a member of a multidisciplinary team.
- Demonstrated organizational skills, time management skills.
- Fluency in Spanish and/ or Portuguese beneficial.
