DIALYSIS EXPERIENCE and LMSW IS A MUST !
Responsibilities of a social worker:
- Facilitating admissions and discharge planning
- Counseling residents and family members
- Completing all required documentation in the medical record
- Including MDS, care planning, discharge planning and being an integral member of the interdisciplinary team.
Requirements:
- BSW minimum
- LMSW
- Long term care and short term rehabilitation/nursing home experience
- MDS and Short Term Discharge planning experience required
- Team player with excellent customer service skills required
