Social Worker

Job Summary
West Covina ADHC Center is seeking a compassionate and dedicated Social Worker to join our team at our Adult Day Health Care Center (ADHC/CBAS). This is a fantastic opportunity to work in a supportive environment that provides essential care and services to older adults and individuals with chronic medical, cognitive, or mental health conditions.

Duties

• Conduct assessments to determine clients' needs and develop individualized service plans.
• Provide counseling and support in areas such as addiction recovery, behavioral health, and childcare.
• Utilize motivational interviewing techniques to empower clients in their personal development.
• Collaborate with community organizations and agencies to connect clients with necessary resources.
• Supervise and mentor interns or junior staff members in social work practices.
• Maintain accurate documentation of client interactions, progress, and outcomes in compliance with regulatory standards.
• Advocate for child welfare and participate in multidisciplinary teams focused on improving client services.
• Stay informed about Medicare regulations and other relevant policies affecting client care.

Experience

• A degree in Social Work or a related field is required; licensure is preferred.
• Experience in workforce development, addiction counseling, or behavioral health is highly desirable.
• Proven ability to work effectively with diverse populations and demonstrate cultural competence.
• Strong interpersonal skills with the ability to build rapport with clients quickly.
• Experience supervising or mentoring others is a plus, contributing to team development and effectiveness.
