JOB SUMMARY
The Enriched Living Center is a 52 bed long-term residence for people with a diagnosis of mental illness. The Resident Care Coordinator will assist the Enriched Living Center program by providing leadership and mentoring to the Resident Counselor I/ II staff. Assist with the overall rehabilitation and the improved quality of life of the residents.
ESSENTIAL JOB FUNCTIONS
- Serve as assigned residents’ primary counselor for service provision.
- Maintain current individual resident records for any assigned residents. This includes meeting with all Primary Counselors to develop and review Service Plans(every 3 months) and Monthly Summaries (30 days)for Residents ensuring they are measurable and completed on time.
- Document progress notes for any residents you have communication or coordinate care with.
- Conduct Resident assessments and Medication assessments
- Check Daily Medication Audits, if not completed communicate with Staff to ensure they are done and they are completed within 24 hours.
- Confirm that Resident Room checks are done on Thursdays and Room cleanings are completed by Mondays.
- Provide assistance in ADL skills.
- Employ as needed (with residents): informal counseling, mediation, conflict resolution, crisis counseling, etc.
- Serve as a member of the ELC On-Call administrative team.
- Assist Residents with Vocational Rehabilitation/Employment refer to the Rescue Mission Community Resource Room as needed.
- Coordinate with direct care staff to implement and maintain monthly activities calendar for residents
- Responsible for Maintenance of resident documentation
- Drive residents in a department vehicle, as needed.
- Assist with the overall rehabilitation that will improve the quality of life for residents.
MINIMUM JOB QUALIFICATIONS
- High School diploma with 5 years of experience with a mental health population and additional pertinent certificates/training in related medical or psychiatric field
OR
- Associate Degree in Human Service or related field with 3 years of experience with a mental health population.
- Minimum of two (2) years management and supervisory experience preferred
- Ability to provide direction/model, delegate, train and supervise others.
- Intermediate skills with Microsoft Word, Excel and Outlook.
- Clean/valid NYS Driver’s License Required

PHYSICAL JOB REQUIREMENTS
The tasks described signify many of the physical requirements of this position, based on a Time Weighted Average of an 8-hour workday.
- Lift/move up to 25 lbs
- Twisting at the torso in stationary position
- Bending at waist using legs to lift heavy objects up to 35 lbs
- Bending at the waist to perform work at that level
- Standing/sitting for extended period
- Walking up and down stairs
- Typing on a computer and using a mouse
- Walking through facility and campus
OTHER WORK PERFORMED
- Driving residents in the Rescue Mission Vehicle e.g. department van.
- Every employee of the Rescue Mission is required to complete annual mandated trainings.
Status/Hours: Full Time, 40 hours per week Mon-Fri 7am-3pm plus On-Call Responsibilities
Pay Rate: $21.00-23.00 per hour, depending on experience
