Job Summary
The Health Home Care Manager is responsible for coordinating and managing care for individuals with complex medical, behavioral health, and social service needs. The Care Manager works collaboratively with members, providers, and community resources to develop and implement comprehensive care plans that promote improved health outcomes and overall well-being.Key Responsibilities:
- Conduct comprehensive assessments to identify members’ medical, behavioral, and social needs
- Develop, implement, and update individualized care plans
- Coordinate services across healthcare providers, behavioral health, and community-based organizations
- Maintain regular contact with members through face-to-face visits, phone calls, and collateral contacts
- Assist members with accessing housing, benefits, and other social supports
- Advocate on behalf of members to ensure appropriate services are obtained
- Monitor member progress and adjust care plans as needed
- Document all interactions and services in compliance with program requirements
- Participate in case conferences and team meetings
- Ensure compliance with Health Home and regulatory guidelines
Qualifications:
- Bachelor’s degree in Human Services, Social Work, Psychology, Nursing, or related field required
- Minimum of 2 years of experience working with individuals with chronic medical and/or behavioral health conditions
- Knowledge of community resources and social service systems
- Strong organizational, communication, and time management skills
- Ability to work independently and as part of a multidisciplinary team
- Proficient in documentation and electronic health records
Preferred Qualifications:
- Experience in Health Home, care coordination, or case management programs
- Bilingual skills (preferred based on population served)
Work Environment:
- Combination of fieldwork and office/remote work (after 90 days)
- Travel required for home visits and community appointments

Pay: $41,000.00 - $54,080.00 per year
