Job Title: PRN Medical Appointment Companion / Care Coordinator

Position Overview:

We are seeking a dependable, compassionate, and detail-oriented individual to support individuals with intellectual disabilities by assisting them with periodic medical and dental appointments throughout the year. This is a flexible, as-needed (PRN) role ideal for a retiree, nurse, healthcare professional, or anyone looking for meaningful part-time work without a fixed schedule.


Key Responsibilities:

  • Schedule and confirm medical and dental appointments on behalf of individuals
  • Provide safe, reliable transportation to and from appointments
  • Accompany individuals during appointments, taking detailed notes and asking relevant questions as needed
  • Advocate on behalf of the individual to ensure their needs, preferences, and concerns are clearly communicated
  • Support individuals with intellectual disabilities in understanding their care and instructions in a respectful, patient manner
  • Accurately document appointment outcomes and recommendations
  • Communicate appointment summaries and updates to the care team
  • Follow up with healthcare providers when necessary
  • Track and report any medication changes or new instructions

Qualifications:

  • Background in healthcare, social services, or caregiving (e.g., nursing, medical assistant, DSP) preferred but not required
  • Experience supporting individuals with intellectual or developmental disabilities is highly desirable
  • Strong communication and organizational skills
  • Ability to take clear, accurate notes and relay information effectively
  • Compassionate, patient, and respectful demeanor
  • Reliable transportation with a valid and clean driver’s license
  • Comfortable interacting with medical professionals and advocating for individuals
  • Ability to maintain confidentiality and handle sensitive information
