With a mission rooted in dignity, equity, and connection, PHC provides medical and behavioral health services at its clinic in Palo Alto, and community-based, case management and housing services throughout Santa Clara County through our New Directions Program.

Summary

Peninsula Healthcare Connection’s New Directions program is currently recruiting a Clinical Case Manager to provide community-based, intensive case management services to individuals with complex medical and/or psychosocial needs who have also experienced homelessness. Through our partnership with The Santa Clara County Office of Supportive Housing, we provide supportive services to ensure stability, self-sufficiency, and overall improvement in quality of life to individuals that have been provided a permanent supportive housing subsidy.

Clinical Case Manager Duties Will Include:

  • Provide in-person, community-based case management services that are strengths-based, client-centered, and trauma-informed.
  • Complete comprehensive biopsychosocial assessments to evaluate needs and create appropriate care plans.
  • Provide crisis intervention for immediate housing needs, medical, mental health and substance use concerns.
  • In conjunction with subsidy providers, assist clients to utilize their assigned subsidy and maintain permanent supportive housing.
  • Assist clients to access basic needs, medical, mental health, and substance use treatment providers, including accompaniment to appointments as needed.
  • Assist clients to function at most independent level possible by addressing ongoing psychosocial needs once clients are stabilized and not in crisis situations.

Clinical Case Manager Requirements:

  • MSW or equivalent required.
  • Minimum two years experience working with populations experiencing barriers such as individuals experiencing homelessness, chronic medical conditions, substance use and/or mental health conditions.
  • Demonstrated ability to develop collaborative partnerships with staff, clients, other professionals, and community members, including individuals from diverse cultural and socio-economic backgrounds.
  • Valid CA driver’s license, reliable transportation, and legally required insurance.

Clinical Case Manager Qualifications and Skills:

  • Knowledge of principles, ethics and best practices of case management, care coordination, social work, and behavioral health services.
  • Experience utilizing client-centered, trauma-informed practices to serve populations experiencing barriers.
  • Principles and methods of interviewing, counseling, crisis intervention, and assessing psychosocial behaviors.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to work effectively and efficiently with minimal supervision.

Pay: $80,000.00 - $90,000.00 per year
