As a Hospice Social Worker (MSW), you’ll provide holistic, patient-centered care—addressing emotional, psychosocial, and practical needs while guiding families through end-of-life decisions with empathy and expertise. Your work will ensure dignity, comfort, and support when it matters most.

✨ What You’ll Do

• Complete comprehensive psychosocial assessments within 5 days of hospice admission
• Identify and address emotional, financial, environmental, and community needs
• Provide grief counseling and emotional support to patients and families
• Assist with:
  • Advance directives
  • Funeral planning
  • Discharge planning
  • Transitions in level of care
• Collaborate with the Interdisciplinary Group (IDG) to coordinate care plans
• Participate in team meetings and care planning discussions
• Connect families with community resources and support services
• Foster a respectful, compassionate environment that honors patient dignity and privacy

What You Bring

• Master’s Degree in Social Work (MSW) from an accredited program
• At least 1 year of hospice experience (required)
• Strong communication and counseling skills
• Ability to navigate sensitive conversations with empathy and professionalism
• Valid Driver’s License & Auto Insurance (field-based role)
• AHA CPR Certification
• Experience with HomeCare HomeBase (preferred)
