Clinical Social Worker (LICSW)
Location: Hyannis, MA
Duration: 26 Weeks
Schedule: Mon–Fri, 8:00 AM–4:00 PM
Overview
Licensed Independent Clinical Social Worker (LICSW) responsible for providing psychosocial assessments, crisis intervention, discharge planning, and care coordination for hospitalized patients and families in an acute care setting.
Key Responsibilities
- Conduct comprehensive psychosocial and risk assessments (mental health, substance use, family, environmental, and safety needs)
- Provide crisis intervention, counseling, and emotional support to patients and families
- Develop and coordinate individualized discharge and aftercare plans with the interdisciplinary team
- Facilitate placement in community services, long-term care, or home-based care
- Collaborate with physicians, nurses, case managers, and external agencies
- Assist patients with insurance, financial resources, and community service navigation
- Maintain accurate documentation in electronic health records
- Ensure safe, ethical, and timely care transitions
Requirements
- Master’s in Social Work (MSW)
- Current LICSW license (state required)
- CPI/MAB certification within 1 month of hire (annual renewal required)
- Strong clinical assessment, communication, and crisis intervention skills
- Ability to work in a fast-paced acute care environment
- Proficient with electronic medical records
Preferred
- Acute care hospital social work experience
- Strong discharge planning and case management background
