Position Overview
We are seeking a warm, professional, and highly personable Community Liaison / Comfort Concierge to serve as a key connection between our organization, patients, families, and community partners. This role blends outreach, relationship-building, and supportive patient/family engagement to enhance the overall experience of care.
This is not a clinical role, but rather a highly relational position focused on comfort, communication, education, and continuity of care in the community setting.
Key Responsibilities
- Serve as a welcoming and supportive point of contact for patients, families, and community partners
- Build and maintain strong relationships with referral sources, facilities, and community organizations
- Conduct community outreach and represent the organization at events, visits, and educational settings
- Provide non-clinical comfort support, companionship, and resource guidance to patients and families
- Assist with onboarding and orientation of new patients to services in a compassionate, clear, and reassuring manner
- Collaborate closely with clinical and administrative teams to ensure seamless communication and continuity of care
- Support family understanding of services, expectations, and available resources
- Identify opportunities to improve the patient and family experience through thoughtful engagement and feedback
- Maintain accurate documentation of visits, outreach, and communications as required
Qualifications
- Background in healthcare, hospice, social services, hospitality, or community outreach strongly preferred
- Exceptional interpersonal and communication skills
- Warm, calm, and professional presence with strong emotional intelligence
- Ability to build trust quickly with diverse individuals and families
- Highly organized with strong follow-through
- Comfortable working independently in the community setting
- Valid driver’s license and reliable transportation required
Preferred Attributes
- 1 + year experience in hospice, palliative care, or senior services
- Strong networking and relationship-building skills
- Natural “people person” with a service-oriented mindset
- Ability to balance compassion with professionalism in sensitive situations

Pay: From $75,000.00 per year
