Job Summary
We are seeking a dedicated and experienced Registered Nurse - Care Transition Manager to join our healthcare team. In this pivotal role, you will coordinate and facilitate seamless transitions of patient care from hospital settings to post-acute, outpatient, or community-based services. Your expertise will ensure that patients receive comprehensive, safe, and efficient care during critical transition periods. The ideal candidate will possess extensive clinical experience across various specialties, including critical care, trauma, emergency medicine, and geriatrics, and will be committed to improving patient outcomes through effective case management and discharge planning. Key Responsibilities:
- Conduct thorough patient assessments to determine individual needs during care transitions
- Develop and implement personalized discharge plans in collaboration with multidisciplinary teams
- Coordinate with hospitals, outpatient clinics, and post-acute care providers to ensure continuity of care
- Monitor patient progress post-discharge to identify potential complications or barriers to recovery
- Facilitate communication between patients, families, and healthcare providers to promote understanding and engagement
- Manage medical records and documentation in compliance with HIPAA regulations
- Utilize diagnostic evaluation data and vital signs to inform care decisions and discharge readiness
- Engage in utilization management and case management activities to optimize resource use
- Educate patients on medication management, follow-up appointments, and health maintenance strategies
- Support hospital experience initiatives aimed at enhancing patient service and satisfaction
MUST HAVE:
- Active, unencumbered Registered Nurse (RN) licensure in Rhode Island
- Med/Surg RN background strongly preferred
- Minimum 5 years of related clinical experience in Acute Care or Case Management
- Knowledge of utilization review, discharge planning, and third-party payor regulations
Strong interpersonal, oral, and written communication skills
- Demonstrated analytical ability, leadership, and effectiveness in team environments
- BLS from AHA
