As one of our Social Workers, you will plan and organize social services for our residents. You will ensure that the medically related emotional and social needs of each resident are met, helping them and their families navigate the transition to and through long-term care.
Key Responsibilities:
- Resident and Family Support: Provide counseling, emotional support, and advocacy for residents and their families. This includes assisting with the adjustment to placement and managing crisis situations.
- Care Planning: Participate in interdisciplinary care plan meetings to address residents' social and emotional needs. Complete initial and comprehensive psychosocial assessments.
- Discharge Planning: Coordinate all aspects of discharge planning to ensure a smooth and safe transition back to the community or another setting, including arranging for home care services, medical equipment, and follow-up appointments.
- Community Liaison: Serve as a liaison between residents, families, staff, and community resources. Maintain a comprehensive list of community services for resident referrals.
- Grievance Resolution: Document and follow up on all resident and family grievances in a timely manner.
Qualifications and Requirements
- License: Must possess a valid Massachusetts Social Worker License (LSW, LCSW or LICSW)
- Experience: A minimum of 1 year of experience in a healthcare setting, preferably with experience in a Skilled Nursing Facility (SNF) or long-term care environment.
- Knowledge: Familiarity with federal and state regulations, including the MDS (Minimum Data Set) and Medicare/Medicaid processes.
- Skills:
- Strong communication and interpersonal skills.
- Excellent organizational and documentation abilities.
- Effective leadership and problem-solving skills.
- Deep empathy and compassion for the elderly.
