Description:

The Social Worker provides and coordinates a wide range of social services, including psychosocial support, to patients and their families. They play a crucial role in empowering patients and families to negotiate with resource providers and make decisions on their own behalf.

Job Responsibilities:

1. Perform and document psychosocial assessments

2. Empower patients and families to make informed decisions

3. Assist clinical staff in understanding family dynamics and developing care plans

4. Maintain accurate and timely documentation

5. Consult and collaborate with other professionals to promote quality care

6. Participate in interdisciplinary meetings and provide support and education as needed

7. Provide psychosocial support and bereavement services to patients and families

8. Attend and participate in Interdisciplinary Group meetings

9. Evaluate bereavement needs and coordinate care with Bereavement staff, when necessary provide bereavement support

10. Perform other duties as assigned

Qualifications:

Masters degree or above in Social Work (CSWE accredited); Bachelors degree may be considered in specific geographic areas

Valid and active FL LCSW license, preferred

Minimum of one (1) year of experience in health care or hospital setting

Familiarity with concepts and practices of hospice and palliative care

Experience in children, family, and group modalities

Previous experience working with an EMR/EHR system

Ability to manage multiple priorities and handle emotional stress effectively

Excellent communication and interpersonal skills, bilingual in Spanish preferred

Valid driver's license and automobile insurance per Company policy

Ability to travel to off-site locations and physically access various healthcare facilities
