Job Summary:
Provides psychosocial assessments, diagnosis and treatment to patients in their place of residence as prescribed by a physician, to assist the patient/family member(s)/caregiver(s) develop effective coping skills in response to the patient’s diagnosis, treatment and/or life situation.
Minimum Qualificaton:
- Current valid state clinical social worker license
- Master’s degree from a school of social work accredited by the Council on Social Work Education
- Two (2) years of post-masters experience in a clinical setting
- Has one (1) year of social work experience in a healthcare setting
- Prior home health experience preferred
- Has reliable transportation
- Able to communicate effectively in English, both verbally and in writing.
Responsibilities:
Assisting other team members in developing the plan of care
Perform accurate and comprehensive assessments
Assistng other team members to understand the significance and impact of emotional issues on the patient’s overall health
Create clinical notes for all encounters with a patient for the patient’s medical record in accordance with Agency policy and applicable regulations
Develop and teach patient and families about community resources
Participate in in-service and training programs offered by Maple Creek Hospice Agency
Ensure that changes in patients’ clinical status are communicated to other healthcare/hospice team members and Agency office staff verbally and in writing in a timely manner
Coordinate care planning and delivery with patient and family/caregiers, other healthcare team members and Agency staff to facilitate optimal patient outcomes
Educate patients, their families/caregivers and other staff as appropriate, in appropriate medical social work modalities and interfventions
Deliver medical social work modalities and interventions appropriate to the patient’s clinical status and needs, and in accordance with the standard of care for the patient’s age
Provide medical social work modalities in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors
Identify risk of injuries or threats to life and health in the patient’s environment, i.e., environmental/ safety/ mobility risks and implement appropriate interventions to reduce those risks
Perform transfer and discharge planning functions in accordance with Agency policy and applicable laws, regulations and standards
Participate in the organization’s information management program
