Social Worker- Director of Case Management

Clinical Magnet (part of ICON Medical) is seeking a dedicated Social Worker leader to serve as Case Management Director at a premier inpatient rehabilitation hospital in Northern Virginia.

Job Summary:

As the Case Management Director, you will lead a team responsible for coordinating patient care, discharge planning, and utilization management. This role is ideal for a Social Worker with leadership experience who is passionate about driving patient-centered outcomes and supporting interdisciplinary care teams.

Key Responsibilities:

· Oversee daily operations of the Case Management department

· Lead discharge planning and care coordination efforts

· Ensure compliance with healthcare regulations and quality standards

· Supervise and mentor Case Managers and support staff

· Collaborate with physicians, nursing, and therapy teams

· Maintain relationships with payors and community resources

· Step in to support caseloads when needed

Qualifications:

· Bachelor’s or Master’s degree in Social Work required

· Licensed Social Worker preferred (LCSW or equivalent)

· Minimum 3+ years of hospital-based case management leadership experience

· Strong experience in utilization review and discharge planning

· Acute care or inpatient rehab experience preferred

· CCM or ACM certification preferred (required within 1 year if not already obtained)

Position Overview:

· Location: Northern Virginia (Aldie area)

· Job Type: Full-Time, Onsite Leadership Role

· Compensation: $120,000 – $130,000 annually + quarterly bonus opportunity

· Schedule: Day shift, flexibility based on departmental needs

· Leadership of established Case Management team
