Position Summary: Responsible for in-home care clients in a geographic area. Hours: Full Time Monday through Friday 9 am - 5 pm

Essential Duties:
- Managing clients: A home care coordinater manages a caseload of potential clients, and may visit them in their homes to assess their needs and well-being. They also ensure that clients receive appropriate care for their evolving health conditions.
- Maintaining records: A home care coordinator maintains records and prepares reports, and may ensure that client and employee records are up to date.
- Ensuring compliance: A home care Coordinator may ensure that care and support is provided in line with regulatory requirements.
A home care Coordinator may also be responsible for: Selecting and assigning staff, Identifying training needs for staff, Developing and implementing care plans, and Maintaining proper labor relations.
Position Requirements & Competencies:
- Must have high school diploma or equivalent.
- Three years of scheduling or home care experience required
- Interpersonal, organizational and communication skills.
- Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.
- Must have reliable transportation.
