Home Care Coordinator cum Receptionist

Position Summary: Responsible for in-home care clients in a geographic area. Caregiver and/or home care experience preferred.

Hours: Full Time Monday through Friday 9 am - 5 pm

Location: 350 W. North Ave. Addison, IL 60101

Essential Duties:

• Managing clients: A home care coordinater manages a caseload of potential clients, and may visit them in their homes to assess their needs and well-being. They also ensure that clients receive appropriate care for their evolving health conditions.
• Maintaining records: A home care coordinator maintains records and prepares reports, and may ensure that client and employee records are up to date.
• Ensuring compliance: A home care Coordinator may ensure that care and support is provided in line with regulatory requirements.

A home care Coordinator may also be responsible for: Selecting and assigning staff, Identifying training needs for staff, Developing and implementing care plans, and Maintaining proper labor relations.

Position Requirements & Competencies:

• Must have high school diploma or equivalent.
• Three years of scheduling or home care experience required
• Interpersonal, organizational and communication skills.
• Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.
• Must have reliable transportation.
