Jennifer Temps is seeking a compassionate and organized Social Worker for a short-term leave coverage assignment in Poughkeepsie, NY. This is an excellent opportunity for a clinician who enjoys working in a school-based, interdisciplinary setting and making a meaningful impact on children and families.
The Social Worker will manage a small caseload of children in an interdisciplinary treatment program serving students in kindergarten through 6th grade. This role includes individual, group, and family therapy; crisis support; discharge planning; collaboration with schools and outside agencies; and participation in a multidisciplinary team environment. The ideal candidate is clinically strong, team-oriented, and comfortable with documentation and care coordination.
Job Duties
- Maintain a caseload of approximately 6 children in the interdisciplinary treatment program
- Participate in admissions with the child, family, physician, and school liaison
- Provide individual therapy weekly
- Provide daily group therapy
- Provide family therapy and crisis intervention as needed
- Facilitate transition meetings for students on caseload, including family and school stakeholders
- Coordinate with participating schools and community agencies
- Function as part of an interdisciplinary treatment team
- Participate in weekly treatment team meetings
- Advocate for youth and families to obtain community-based supportive services
- Maintain timely and accurate medical record documentation
- Support discharge planning and complete discharge follow-up
- Collaborating with school liaisons throughout Dutchess County to support referral and screening processes
Minimum Qualifications
- Master’s degree in social work
- New York State Social Work license required
- Prior experience providing therapy to children and families
- Experience with crisis intervention, case coordination, and discharge planning
- Strong documentation and organizational skills
- Ability to work collaboratively within an interdisciplinary team
Preferred Qualifications
- Experience in a school-based, behavioral health, or child-focused treatment setting
- Experience facilitating group therapy
- Familiarity with community resources and school systems in Dutchess County
- Strong communication skills with families, school personnel, and outside providers
Pay Range: $38 – $45 per hour (depending on experience)
