About the RoleAs an RN Care Manager in Home Health, you’ll serve as the vital link between patients, families, and the healthcare team—ensuring coordinated, high-quality care in the comfort of patients’ homes.Important: This is HOME HEALTH (not hospital) — prior experience is required. What You’ll DoLead and coordinate patient care across a multidisciplinary teamDevelop, implement, and adjust individualized care plansCollaborate with physicians and healthcare providers to optimize outcomesGuide patients and families through care decisions and transitionsMonitor treatments, diagnostics, and resource utilizationSupport complex cases including chronic conditions and high-utilization patientsEnsure compliance with regulatory and accreditation standards What You BringRequired:Active Registered Nurse (RN) license (California or eligible)Home Health experience (MUST HAVE)HCHB (Homecare Homebase) software experience (REQUIRED)OASIS documentation experience (REQUIRED)BLS certificationPreferred:Bachelor’s Degree in Nursing (BSN)Case management experience or certification Important RequirementsAll associates must meet vaccination requirements, including:COVID-19 (required in CA)Influenza (seasonal)MMR and other standard immunizations(Medical/religious exemptions may apply.)
